How To Refer

Thank-you for choosing Mullan Gallagher Dental Care to provide implants for your patient.

To make a referral please complete the form below, which will be emailed directly to us.

Alternatively you can print out the downloadable PDF referral form and post it to us.

We ask you to kindly forward up to date contact and personal details for your patient including, where possible, current e-mail addresses for you and them, to allow for better communication. Please also forward copies of any relevant radiographs.

Referred patients can normally be seen within two weeks of referral.

We look forward to providing high quality implant care for your patients.
